Your First Visit
A patient’s first visit to our office is an exciting one, as it’s where our team gets to know you and your oral health goals. Before your first visit, we recommend filling out and submitting your new patient forms to streamline your appointment. If you are planning on using dental insurance, please remember to bring your insurance card and a valid form of I.D. with you. If you would like any X-rays or other documents forwarded to our office, we ask that you have your previous dental office send them to us prior to your first visit so they are available for us to review beforehand.
